Informed Patient Consent

1. Scope of Clinical Services

At MSM Pro Derma Polyclinic, our priority is providing safe, medical-grade, and scientifically validated aesthetic treatments. This Informed Consent document applies to all clinical procedures conducted in our facility, including but not limited to: laser therapies (laser hair removal, fractional laser resurfacing, vascular lasers), medical injectables (dermal fillers, neuromodulators/Botox, skin boosters), clinical chemical peels, advanced facials (Hydrafacial, medical microneedling), and digital diagnostic procedures (such as the AURA Skin Analysis and 3D digital passports).

Before undergoing any treatment, you will undergo an in-depth diagnosis with one of our licensed healthcare practitioners to design your specific Skin Journey blueprint.

2. Mandatory Medical Disclosures

For your medical safety and to prevent severe contraindications, you are strictly required to disclose your complete medical history. You must inform your practitioner if you have, or have had, any of the following conditions:

• Active Medications: Oral retinoids (Isotretinoin / Accutane) within the past six (6) months, blood thinners (aspirin, anticoagulants), or photosensitizing antibiotics.
• Pregnancy and Lactation: If you are currently pregnant, planning a pregnancy, or breastfeeding (certain clinical injectables and chemical acids are strictly contraindicated).
• Medical Conditions: A history of keloidal scarring, abnormal wound healing, active skin infections, open wounds, eczema, psoriasis, autoimmune disorders, or a history of herpes simplex virus (cold sores) near the treatment site.
• Recent Procedures: Any cosmetic injections, laser procedures, surgical procedures, or deep chemical peels performed within the last four (4) weeks.

3. Common Risks & Side Effects

All clinical aesthetic treatments carry some inherent risks. While our certified medical team takes every precaution to minimize complications, you acknowledge and accept that the following temporary side effects are common and expected:

• Mild to moderate redness, erythema, localized swelling, and tenderness at the treatment area.
• Bruising, small hematomas, or localized petechiae (common after injectables, mesotherapy, or microneedling).
• Temporary skin peeling, dryness, flaking, or superficial crusting (common after chemical peels or laser treatments).

Rare but potential clinical risks include: post-inflammatory hyperpigmentation (PIH) or hypopigmentation (changes in skin color), superficial scarring, secondary bacterial or viral infection, allergic reaction to topicals or injectables, nerve sensitivity, and extremely rare complications like vascular occlusion (blockage of blood vessels, which requires immediate clinical reversal).

4. Patient Pre- and Post-Care Compliance

The success of your Skin Journey and the prevention of pigmentary changes depends heavily on post-treatment care. By proceeding with treatment, you agree to:

• Strictly apply broad-spectrum mineral SPF 50+ sunscreen daily and avoid direct sun exposure or tanning beds for the duration advised by your practitioner (typically 2 to 4 weeks post-treatment).
• Discontinue the use of active homecare products (such as Retinol, Glycolic Acid, Salicylic Acid, or other chemical exfoliants) for 3-5 days before and after your clinical sessions, as directed.
• Refrain from picking, scratching, or peeling any scaling skin or crusting that develops naturally.
• Contact the clinic immediately if you experience severe pain, spreading skin discoloration, blisters, or signs of systemic fever.

5. Individual Outcomes & Results

Human skin is a complex biological system, and clinical response varies based on genetics, lifestyle, age, metabolic rate, and compliance with home care. While MSM Pro Derma utilizes state-of-the-art medical technology and follows proven clinical protocols, there is no guarantee of specific, permanent, or absolute results. Multiple sessions are frequently required to achieve optimal therapeutic goals.

6. Consent Acknowledgment

By signing the physical consent forms at our clinic lobby and checking the diagnostic confirmation box on your digital Skin Passport, you confirm that:

• You have read and fully understood the information provided in this consent framework.
• You have had the opportunity to discuss the treatment plan, alternative options, and expected outcomes with your doctor.
• You voluntarily authorize the medical practitioners and clinical therapists at MSM Pro Derma Polyclinic to perform the selected procedures.
